Thank you for scheduling an appointment with us. Feel free to ask about our New Patient special and our Belterra membership program. An appointment at our Practice is a reservation with the Doctor, Dental Assistant & our Hygienist. Each appointment is scheduled for a specific amount of time based on our patients needs. In order to reserve a scheduled appointment, Office Policy requires a deposit towards pending treatment at the time an appointment is scheduled. If you are utilizing dental benefits, please provide your insurance information 48 hours prior to your visit. Please be aware of cancellation policy. Our Practice requests patients to give us a 48 hour notice in order to cancel appointments. Patients who are unable to provide a 48 hour notice to change or cancel their appointment will be subject to a $50 Service Charge. This policy enables us to put our patients first, and deliver dedicated comfort centered treatment.
